The name of the buyer goes in this blank. All parties that want to be on the title should be named here. Using the name (and/or initials), as they want it to appear on the title.

The name of the seller goes in this blank. All parties that are on title should have their names here, the same way they appear on the title. You can use the term Owner of Record and the Closing Agent should pick up and use the owner’s name that is recorded on the title.

The county that the property is located in goes in this blank.

If you have the Legal Description put it in this blank. If it is a lengthy description you can write in See Addendum “A” and attach a legal description. If you do put one in here be sure it is exactly as it appears on public record. It is not mandatory that a description be put in here.
